Ubuntu and PHP release security patches regularly. An unmanaged VPS runs on whatever was installed at setup - often months or years out of date. Known vulnerabilities, unpatched.
Logs grow. Temp files accumulate. MySQL binary logs fill the disk. One day the disk hits 100% and the server stops accepting writes. Your site throws 500 errors. Nobody was watching.
